Can't reproduce this at the moment. It has a 100% chance to drop from the Universal Folders: Redeemed/ Unredeemed - so it should never be empty. Can you provide a screenshot of the issue happening…
It's related to Nano Recharge.
While on Nano Recharge, activating an Item seems to hinder a usable item from applying it's effects successfully.
Unfortunately we didn't get any further information from .bug (and pasting the generated link) or even a getfull to test/repro properly.

It looks like higher QL items are still dropping from SL Dynas: QL 300 "Die Nadel" and 2) Level 59 Symbiant (QL 80) from a Level 15 SL Dyna).
